Session: AGING SKIN: THE LATEST EXTRINSIC & INTRINSIC METHODS FOR REJUVENATION In our clinic we use two new technologies for skin anti-aging treatments. 1) For treatment of 240 patients we combine PRP with preparation of Autologus Trombine-fibrin Serum, contains activated thrombin (15.45 ± 5.39 U/ml),which catalyse conversion of soluble fibrinogen to the gel-like substance - fibrin, the major poteintiating component of the clot. The coagulation cascade, when acutely stimulated, can convert the entire plasma content of fibrinogen to fibrin and force the level of circulating platelets to zero in less than a minute. Polymerized fibrin mesh with activated platelets, trapped inside, releasing a big quantity of cytokines and growth factors. Aging and age related degenerative diseases are characterized by a pro-oxidative, pro-inflammatory state that may contribute to their physiopathology, because of the deleterious side attacks of free radicals on cell constituents and on the connective tissues. The brain is an organ particularly likely to oxidative damage. The cerebral metabolism requires high levels of energy and is closely dependent on aerobc conditions; is also very rich in polyunsaturated fatty acids, easily oxidizable and transition metal, likely to facilitate the formation of radical OH-. In addition the brain is characterized by a low content of antioxidant systems compared to other tissues and organs and is therefore particularly susceptible to oxidative damage. Thus the alteration of oxidative metabolism present in old age might make the brain tissue more susceptible to damage caused by the accumulation of neurotoxic peptides such as beta-amyloid. Studies on brain tissue of AD patients post-mortem have shown an alteration of several oxidative damage markers, including an increase in lipid peroxidation, oxidative damage of proteins, and glyco-oxidation, and a reduction of antioxidant enzyme systems. Thus AD seems to be associated to an increased oxidative stress via several mechanism: a) an increase in production of reactive oxygen metabolites, b)a decline of the antioxidant systems and defence and c) a decreased efficiency in the repair of damaged molecules. We studied a group of AD patients in our Alzheimer Evaluation Unit. All patients were tested for measurement of 8-OHdG in urine to assess the importance of evaluating oxidative stress, and we found a significant elevation of oxidative stress in our AD patients, compared to age-matched non AD controls. Many of these problems can be successfully approached with medical techniques, which consist of using only hyaluronic acid for the correction of nasal angles and botulinum toxin for patients with hyperactivity of the depressor septi nasi muscle and those with bunny lines. One hundred non-smoker patients aging between 25 and 55 years were included in this study. The method of analysis developed involves the use of vertical and horizontal lines to divide the nose into areas. we treated these patients with HA (volume), Allergan) and onabotulinum toxin A (VistabexAllergan) Postsurgical analysis with nasal grid indicated a satisfactory outcome and it proved useful for precise records. The overall rate of complication was low. After very easy fat harvesting with a local anaesthesia, we have to avoid having fibres inside. The fat is prepared as a milk shake, and very easy to inject. A thin canula can run on the complete external side of the hand without any injury and let the fat milk shake everywhere. One procedure is most of the time enough .The fat gives a nice form, covers the veins and vessels, and some of the tendons. The PRP method to regenerate the conjunctive part of the skin, was coming in 2007, and I use to prepare the patient with it first. Why? HRV is the conventionally accepted term for describing oscillation in consecutive RR intervals of cardiac cycles. Evaluation of cardiac autonomic regulation by means of HRV is a non-invasive, simple and frequently used measure, which has proved successful in experimental research as well as in clinical or sport practice. HRV can be influenced by numerous factors with age, diseases or exercise ranking amongst the most investigated. Evidence-based data exist about the close relationship between HRV and a number of civilizational diseases. Rapamycin was discovered in the soil of Easter Island. Mechanistically, rapamycin binds FKBP12, an immunophilin with prolyl isomerase activity. Two additional proteins required for itseffects in yeast were identified in a genetic screen in 1991 and termed target of rapamycin 1 (TOR1) and TOR2. Reduced TOR signalling has been shown to significantly increase lifespan in a variety of organisms. Rapamycin, an inhibitor of mechanistic target of rapamycin (mTOR), has the strongest experimental support to date as a potential anti-aging therapeutic. Rapamycin extends life span in yeast, worms, flies and mammals. It was recently demonstrated that long-term treatment with rapamycin, or ablation of the mTOR target p70S6K extends both mean and maximum lifespan in mice, possibly by delaying aging. Rapamycin extended median life span in males and females by 10% and 18%, respectively, and maximum life span by 16% and 13%. However, the mechanism that accounts for these effects is far from clear. Longevity effects of rapamycin may, however, be due to rapamycin’s effects on specific life-limiting pathologies, such as cancers, and it remains unclear if this compound actually slows the rate of aging in mammals. Cancer is the most common cause of death for laboratory mice, and rapamycin is an anticancer drug. Therefore, it remains possible that life span extension by rapamycin is secondary to tumour suppression and unrelated to the underlying aging process. However, many other potential mechanisms of life span extension are possible. mTORC1, via S6K and 4E-BP, plays a central role in the regulation of translation, and it is worth considering whether reduced protein synthesis per se might mediate the effects of rapamycin on longevity. Another effect of mTOR inhibition that has been linked to longevity is the induction of autophagy, a process by which cells recycle their proteins and organelles. Autophagy allows cells to survive nutrient-limited conditions and is a central mechanism by which damaged components are removed. Rapamycin has a number of interesting effects on stem cell function. Inspections were carried out at 5, 10 days and in 1, 6 and 12 month time. In most cases, procedures were well tolerated. The severity of procedure pain and after treatment burning sensation were ranged from 0 to 3 and from 1to 4 on the British pain scale respectively. 105 patients required a single dose of oral analgesic (Ketorolac, 10 mg) to relieve the feelings. There habilitation period presented by erythema during the first day, followed by the epidermis superficial layer exfoliation was an average of 5 days. Side effects were not observed. An instrumental assessment of hand skin hydration (by Corneometry) and elasticity (by Cutometry) was performed before injection sessions on D01 and on M01, M02, and on M03, M04, M05 and M12 after the first injection session. All patients alsofilled out a satisfaction questionnaire. Results The mean age of the 27 women that were included in the study was 53 years (range: 42-65 years). Efficacy of treatment assessed by the patient using GAIS on M12 (2.0±0.5) was comparable with the assessment on M02 (2.2±0.6).Three treatments with stabilized HA significantly improved skin elasticity. Hand skin became more elastic on M03, this effect was maintained at M12compared with D01. Hydration of hand skin remained stable during the observation period. According to the satisfaction questionnaire the best result of self-assessment was observed on M05. Hand skin looked younger and less wrinkled; this condition was stable during the observation period. All patients would like to repeat the procedure and would recommend this treatment to their friends. The result of the self-assessment on M12 was comparable with the results on M01-M05.ConclusionWe have the first prospective Russian series of microinjections of stabilized hyaluronic acid for hand skin rejuvenation and observed that long-term results and patient’s satisfaction were good.
